Access and Equity Services (AES) Pre-Intake Form

Student Information

To begin the registration process with Access and Equity Services (AES) you must first complete this form.

Once AES has reviewed your form submission, AES will contact you via an email sent to your USask email address advising you to request a registration appointment.

Submission of this form does not mean you have registered with AES.

AES Registration Deadlines

To have accommodations in place for final exams, your documentation must be submitted by the following AES registration deadlines:

• Fall Term: November 10 at 4:30 pm
• Winter Term: March 10 at 4:30 pm

Documentation received after these dates will still be reviewed, but accommodations may not be in place for the upcoming final exam period.
