To begin the registration process with Access and Equity Services (AES) you must first complete this form. Once AES has reviewed your submission, we will contact you via your USask email address advising you to request a registration appointment.
Submission of this form does not mean you have registered with AES.

ℹ️ To register with AES for a disability-related reason, we require either a completed AES medical questionnaire OR a psychoeducational assessment that is less than 5 years old.A government Verification of Disability form is not accepted on its own.
